Is Dubuque Physical Therapy a Blue Shield Alliance select provider?

Yes, Dubuque Physical Therapy is a free standing out-patient clinic.  Therefore, the deductible is waived.  The patient is only responsible for co-insurance.  If the patient would seek care from a hospital or a hospital affiliated clinic, the patient would be required to pay a deductible.

For example:

If a patient was seen for 6 visits and charges totaled $500.00 the difference would be:

 

Hospital or Hospital Affiliate

 

 

Dubuque Physical Therapy

 

Patient would be required to pay:

$500.00

 

 

 

Patient would pay:

6 visits $10.00 co-pay = $60.00

6 visits $15.00 co-pay = $90.00

6 visits $20.00 co-pay = $120.00

$500.00 x 10% co-pay = $50.00

Does my insurance require pre-certification for outpatient physical therapy?

 All insurance requirements are different. However, at your request the Patient Accounts Representative would be happy to contact your insurance carrier to inquire pre-certification requirements.

If my insurance company requires pre certification, do I need to call my insurance carrier?

 No, our Patient Accounts Representative will contact your insurance carrier for pre-certification.

 Is it necessary to have a doctor’s prescription for physical therapy?

 A licensed Physical Therapist may provide treatment without a doctor’s prescription. However, your insurance may require a prescription from your physician for physical therapy to be a covered benefit. All insurance requirements are different. The Patient Accounts Representative would be happy to contact your insurance to inquire about requirements.

What if I have no insurance coverage?

You can see a Physical Therapist without insurance or a physician prescription. 

Our clinic offers up to a 40% discount for all cash based accounts. Please contact Patient Accounts Representative for details.

Does my insurance cover physical therapy?

 In most cases physical therapy is a covered benefit. At your request, the Patient Accounts Representative will contact your insurance carrier to verify benefits.
